Criteria
For Graduation From Phase II Intensive Outpatient Treatment
to Discharge or "Monitoring"

Clients have no toxic Withdrawal Risks or Problems
- Clients are beginning to demonstrate that they can identify
Post Acute Withdrawal Symptoms (PAW) and the client has demonstrated
that his/her experiences with those symptoms are manageable with
one time weekly contact/support. (Outpatient Group Treatment) |

Biomedical Conditions
are stable and provide no distraction from Outpatient Treatment.
Clients have demonstrated that they understand their need for
balanced nutrition and are beginning to develop and implement
nutritional management techniques. |

Emotional/Behavioral Conditions
are beginning to stabilize and Clients are beginning to demonstrate
that they can manage recovery issues without being overly emotional.
Clients are beginning to demonstrate an ability to see situations
approaching that would affect their thoughts, feelings, emotions
and/or behavior and are beginning to intervene on those situations
for themselves. |

Treatment Acceptance/Resistance
is managed and Clients have demonstrated that they understand
their need for treatment and are demonstrating active involvements
in treatment but continue to need structured support. |

Relapse Potential
is reduced because Clients have demonstrated an understanding
of how they can personally manage High Risk Situations (HRS) and
the associated relapse symptoms and are demonstrating willingness
and some capability to develop and implement High Risk Situation
management techniques for themselves |

The Recovery Environment is
stabilized because Clients have demonstrated an ability to personally
manage and develop a safe living environment and they
are beginning to demonstrate an ability to connect socially outside
of their home environment. |

have no toxic Withdrawal Risks or Problems - Clients have
demonstrated that Post Acute Withdrawal Symptoms (PAW) are client/self-manageable. |

#2
|
Biomedical
Conditions are stable. Clients have demonstrated the ability to balance their nutrition
and have implemented nutritional management techniques. |
|

|
Emotional/Behavioral
Conditions are stable and Clients are demonstrating that they can manage their
emotions and behavior and are comfortable intervening in emotionally
charged situations for themselves. They are not typically surprised
by those situations and have demonstrated that they are cognitively
able to manage the emotional situations that confront them. |

Treatment
Acceptance/Resistance is managed and Clients have demonstrated that they understand how
use of Alcohol, Drugs or old behavior affects their lives and
have demonstrated their ability to reach out for professional
help as needed should continued support be necessary. |

Relapse
Potential is reduced significantly because Clients have demonstrated
an ability to personally intervene in relapse situations and appropriately
use their personally developed High Risk Situation (HRS) and Relapse
Warning Sign intervention techniques without regularly structured
professional support. |

Recovery
Environment is stable because Clients have successfully demonstrated their ability to
personally protect and manage their safe living environment.
Clients have successfully demonstrated their ability and willingness
to build an outside social support system/network. Clients also
have in place a working, interactive Relapse Intervention
Team, have met with them in supervised sessions, and have developed
a personal schedule for that team to meet when they are no longer
in Outpatient Treatment. |
